* If you are scheduled to work between 32 and 79 hours per pay period, you are eligible to participate. However, cost sharing may increase for the employee.

All employees are eligible for miscellaneous benefits such as Workers' Compensation, Direct Deposit, U.S. Savings Bonds, Jury Duty, Credit Union, Free Parking and appropriate Professional Dues.

Health Care Coverage

SCLHS offers three health care options.The Base PPO Plan and Plus PPO Plan provide benefits both in-and out-of-network. Annual deductibles, co-payment levels and out-of-pocket maximums vary based on where services are received and the plan you select.The Supplemental Plan is designed to "tie in" with another plan the participant may have. This plan pays the first $500 in covered expenses and then pays 20% of remaining expenses. 

When you are eligible: First day of the month following employment 

Cost: SCLHS pays 80% of the total monthly premium for the Base PPO Plan; may elect to "buy-up" to Plus PPO Plan.

Dental Care Coverage

The SCLHS offers three dental care options. The $25/$50 option pays 100% of preventive and diagnostic services. Lifetime maximum of $2,000 for orthodontics and TMJ. The $50/$100 option pays 100% of preventive and diagnostic services. Lifetime maximum of $1,000 for orthodontics and TMJ. CIGNA Dental Health option is a dental maintenance organization plan. 

When you are eligible: First day of the month following employment 

Cost: SCLHS pays 80% of the total monthly premium for the $50/$100 plan; may elect to "by-up" to $25/$50 plan.

Vision Plan

This benefit covers annual vision exam plus eyeglass and/or contact lenses every year and new frames every two years for each covered participant. The plan pays 80% of eligible costs up to maximum of $250/person.

Flexible Spending Accounts

The Health Care Spending Account is for health care costs not reimbursed by medical, dental and/or vision insurance. The Dependent Care Spending Account is to pay for eligible expenses incurred for childcare for dependent children under the age of 13, or, other elder care as defined by the IRS. 

When you are eligible: First day of the month following employment  

Cost: You may defer pretax up to $5,000 annually for health care expenses and up to $5,000 annually ($2,500 if married and filing separate returns) for dependent care expenses.

Life Insurance

FULL TIME: The plan provides the opportunity to purchase up to five times your annual base salary in life insurance, up to maximum of $1,000,000. 

PART TIME: The plan provides $15,000, $25,000 or $50,000 coverage.

When you are eligible: First of the month following employment 

Cost: For full-time, SCLHS provides for two times your annual base salary, with an option to purchase an additional three times annual base salary up to maximum of $1,000,000.

AD&D Insurance

FULL TIME: The plan provides the opportunity to purchase up to five times your annual base salary in AD&D coverage, up to a maximum of $1,000,000.

PART TIME: The plan provides $15,000, $25,000 or $50,000 coverage.

When you are eligible: First of the month following employment 

Cost: For full-time, SCLHS provides for two times your annual base salary, with an option to purchase an additional three times annual base salary up to maximum of $1,000,000.

Dependent Life Insurance

The plan provides three different options. The options include:$10,000 spouse/$5,000 child;$20,000 spouse/$10,000 child; or$50,000 spouse/$25,000 child. The amount of dependent life insurance cannot be more than your life insurance coverage. 

When you are eligible: First of the month following employment 

Cost: You pay 100% of the premium, which is paid on an after-tax basis.

Long Term Disability

The SCLHS offers three LTD options that provide salary continuation of 40%, 50% or 60% of monthly salary up to a maximum benefit of $6,000, $7,500 and $9,000 respectively. Benefits are integrated with Social Security, and payments begin following 90 days of disability and may continue until age 65. 

When you are eligible: First of the month following employment. 

Cost: SCLHS pays 100% of the premium

Long Term Care

This benefit is designed to help cover the costs associated with extended long term care, such as professional in-home care, assisted living facilities and skilled nursing facilities. This plan is available to you and your extended family members. 

When you are eligible: First of the month after receipt of approval from carrier. 

Cost: You pay 100% of the premium.

Paid Time Off (PTO)

Accrual rate based on hours paid and length of service with a maximum accrual of 50 days or 400 hours. 

When you are eligible: Upon employment 

Cost: SCLHS pays full cost.

Extended Illness (EI)

Extended illness is paid starting on third day of each illness. Accrual rate based on hours paid with a maximum accrual of 90 days or 720 hours. 

When you are eligible: After three months of employment, may use EI for personal illness, injury, or pregnancy. 

Cost: SCLHS pays full cost.

Bereavement Leave

Depending on family relationships, granted paid leave in the event of the death of a family member.  

When you are eligible: After three months of employment.

Defined Contribution Plan

The SCLHS Defined Contribution Plan provides a 5% basic employer contribution toward the participants retirement needs and to encourage individual savings for retirement. SCLHS provides a 50% matching contribution of first 6% a participant contributes to the Retirement Savings Plan. The vesting schedule is as follows: 3 years - 50%, 4 years - 75%; 5 years - 100%. 

When you are eligible: Eligible to participate after one year of service in which 1,000 hours worked.  

Cost: SCLHS pays 100% of the cost

Retirement Savings Plan

The SCLHS' Retirement Savings Plan is designed to provide an opportunity for employees to save toward their  retirement Any employee may request a portion of their income to be withheld and invested in the Plan rather than being paid as taxable income.  

When you are eligible: At the time of employment 

Cost: You may contribute a portion of your pay on a pre-tax basis, up to the IRS maximum contribution limit.

Educational Assistance

The benefit is to encourage employees to pursue professional development. Educational assistance may be used for tuition, fees and books. The course or degree program must be offered by an accredited educational institution. Graduate level courses may be reimbursed but are subject to applicable federal and state taxes. 

When you are eligible: After one year of employment 

Cost: SCLHS reimburses up to $1,000 per year

Leave of Absence

An unpaid leave of absence (LOA) may be granted for education, medical, military or personal reasons for any eligible employee not on disciplinary probation. 

When you are eligible: After one year of employment, may be granted unpaid time off without loss of seniority.

Employee Assistance Program

The EAP is voluntary and strictly confidential. The program is to assist you with persistent problems or issues that may affect you at home or at work.  

When you are eligible: At the time of employment 

Cost: SCLHS Pays 100% of the cost
